IMPORTANT:

• To accommodate social distancing recommendations, this program is being conducted as a drive-thru event.
• This is a SALIVA test where you will need to collect your saliva in a specimen tube, so please DO NOT eat, drink, smoke, vape or chew gum for at least 30 minutes prior to your test. In doing so this could affect your testing time.
• Please wear a mask.
• Optional: Please bring a working smart phone or tablet with you, if possible. It will help us maintain recommended social distancing regulations.

APPOINTMENT REGISTRATION SITE

PLEASE NOTE THE FOLLOWING:

1. When completing the information below, please provide the name of the decedent and type of service you will need in the field titled Service Type.
   1. For Example – Probate Will, John Smith, deceased; Administration, Barbara Jones, deceased
2. To accommodate social distancing requirements, the only parties to attend your appointment will be the executor/administrator or individual seeking guardianship and 1 support person. No children will be permitted.
3. The Camden County Surrogate Court can only accept check, cash or money order as forms of payment.  We do not accept credit or debit cards.
4. In order to have the proper documentation with you, for your appointment, Please visit our website at: https://www.camdencounty.com/wp-content/uploads/2016/04/cover-letter-for-probate-by-mail-2023.pdf, to secure the information and documents required.
5. To proceed in Camden County, the decedent must have resided in Camden County. Please be sure to have documents which refer to the Camden County address.
6. In order to assist you, the Camden County Surrogate Court has three locations: Our main location in Blackwood and our satellite offices are in Voorhees Township and Camden City. Please select the location you wish to appear and make an appointment.

7. If you are not able to make your scheduled appointment, please contact our Court at 856-225-7282 to cancel.
